Services for Graduates and Guests with Disabilities Form

2008 Commencement Ceremony - Thursday, June 12 at 7:00 p.m.

If you are a graduate or guest of a graduate in need of special arrangements due to a disability, please contact Jean Carlson at (253) 833-9111 ext. 2631.

Designated seating is available for guests with disabilities and mobility issues (i.e. limited mobility [wheelchair/walking stick], hearing impaired, back injury, etc.). To request a reasonable accommodation for a graduate or guest(s), please submit the Services for Graduates and Guests with Disabilities Form below. All requests must be submitted by Friday, June 6. Special arrangements can not be guaranteed after June 6.

By completing this form, you are requesting Assisted Seating. Therefore, you will need to see Jean Carlson at Disability Support Services, located on the Main Campus, Lindbloom Student Center, Room LC-271, for Assisted Seating tickets. Guests with disabilities must show their Assisted Seating ticket upon arrival to the ceremony. Thank you for allowing us the privilege of helping you and your guests enjoy this memorable evening.
