Phlebotomy Disqualifying Factors

A criminal record may possibly prohibit a student from obtaining state licensure. The Washington State Department of Health will obtain background checks for licensure purposes through the Washington State Patrol and the Federal Bureau of Investigation. While, Green River does not run a criminal background check on students, they are highly encouraged to visit: https://www.dshs.wa.gov/sesa/background-check-central-unit/disqualifying-list-crimes-and-negative-actions for a list of certain criminal convictions, pending charges, and negative actions that automatically disqualify a person from having unsupervised access to vulnerable adults, juveniles, and children. Green River is not responsible for students who cannot license due to criminal convictions, pending charges or negative actions.

Students who are interested in completing the Phlebotomy Certificate need to complete the MA-Phlebotomy Background Attestation Form and turn it in at the mandatory orientation session.
